In the south-east, in Haut-Mbomou prefecture, the army is fighting the Azandé Ani Kpi Gbé militia; aid can reach the area only by air.

The 2026 humanitarian plan covers 2.3 million people in need and targets 1.3 million, requiring 264.2 million US dollars. In August 2025 there were 442,320 internally displaced people, and the country hosts more than 52,700 refugees, mainly from Sudan, Chad, South Sudan and the DRC. In January 2026 fighting in Zemio forced about 2,000 people to shelter in the hospital and a church, short of food, water and medicine.
